Green Card / Form I-693:
Immigration medical exams are required for all persons applying for a US Green Card. Immigration health exams are also called Change of Status Exams, or an I-693 exam. This health screening MUST be performed by a USCIS Civil Surgeon.
To facilitate efficient processing of U.S. immigration physical exams, all applicants must provide the following:
• Picture identification (2 by 4 inch required). (i.e. Driver’s License, State ID)
• Passport or any other Government issued photo ID.
• Vaccination records.

Medical Certification for Disability Exceptions / Form N-648
The USCIS grants medical disability waivers to applicants who are medically impaired with an extreme condition or combination of conditions such that they are unable to learn or demonstrate knowledge of English and/or U.S. history and government.
In order to process a Medical Certification for Disability Exceptions, the applicant must provide the following:
• Alien Registration number (starts with an “A”).
• Supporting medical records
